Are you looking for information or support for the memory loss journey? At the Memory Hub, a Social Worker from Kin On Health Care Center acts as “Memory Navigator” to provide consultation about important topics. They provide free 30-minute appointments to help you get connected with helpful memory loss resources.

 In-person appointments are available at either 1pm or 2pm on the following Mondays: 

• Sept 8: 1pm or 2pm 

• Sept 15: 1pm or 2pm 

• Sept 29: 1pm or 2pm 

• Oct 6: 1pm or 2pm 

• Oct 13: 1pm or 2pm

What can I discuss with the Memory Navigator?

What other services can the Memory Navigator connect me to?

  • • Legal planning 
  • • Financial counseling 

 

Do you prefer connecting over the phone? 

You are welcome to call the Alzheimer's Association 24-Hour Helpline to speak for free to a Master's level social worker, any day or time. The number is 1-800-272-3900. Note that the Helpline is a good fit for people or caregivers looking for resources about any memory-related condition, not just Alzheimer's.
